How bad does it have to be before I see a doctor about hemorrhoids ?

IF they are extreemly painful, you might have a thrombosed hemorrhoid. IT might or might not resolve. The pain is excruciating, and you likely would make the choice to go in.
They are impacted with blood, and usually the tissue is discolored. External "piles", are a common problem, and some do bleed. You will always be advised to see a doctor to rule out other more serious issues, when bleeding is present.
They say diet and exercise, and adequate intake of fiber helps. I think exercise is on top of the list.

most likely if you are asking this question it is bothering you enough to need to see a doctor. but usally the rule is if the start bleeding and you cant stop the bleeding then it is time to see the doctor but it is always good to go to the doctor and let them know what is going on.
