It is completely location. Sciatica tends to go near the outside of your buttocks and shoots down your leg where as hemorrhoids are sore where they are located right around or in the rectum area. Internal hemorrhoids do cause pain and they do not always bleed.

Internal hemorrhoids don’t hurt, they just bleed.
External hemorrhoids hurt.
Sciatica is pretty characteristic pain that shoots down your buttocks into your leg. External hemorrhoids hurt when you sit, wipe etc.
