sometimes they have to be surgically treated if nothing helps

Call your doctor and they can call in some prescription strength suppositories.

Edit: Yep, yep. I read that. With a suppository, they melt. Thus coming out of the rectum. You may have better luck with that than trying cream that may come off in your undies. :)

Ask for a stronger prescription strength hemorrhoid cream. Also use Tucks (witch hazel) pads. Increase your fluid and fiber to avoid straining with a bowel movement and prevent them from worsening. That’s about all you can really do.

The first treatment of hemorrhoids is often with a high-fiber diet and lots of fluids to promote soft regular bowel movements. Cleansing and toilet habits are important. Toilet paper use after a bowel movement may be abrasive. Wiping with synthetic cotton swabs premoistened with warm water may be more comfortable. Vigorous rubbing should also be avoided. Take a sitz bath by soaking in warm water several times a day, especially after having a bowel movement, to keep the area clean.

    TUCKS WITCH HAZEL PADS.
AND SOAKING YOUR BUTT IN WARM CLEAR WATER FOR 1/2 HOUR 3 TIMES A DAY.

IF THEY BLEED OR SEEM REALLY SORE AND PAINFUL,SEE A DOCTOR.

Curly Sue is right. The suppositories melt and they’re very effective on the external type too. Additionally, they relax the area so that you don’t strain too hard for a b/m.

My top tip is to have a ‘sitz bath’ 3 times a day. Run a few inches of warm/hot water and sit in it. Very calming, and helps to remove bacteria/ If you can deal with it, salt will help, but it hurts like hell the first time.

Tucks are good, but a bottle of plain old Witch Hazel is the same stuff for a fraction of the price; you just moisten the toilet paper with some before you wipe.
