You must not hold your breath and grunt, and strain when you go. there are 3 veins around the rim when one or all get strained outside, then they get restricted and don't let the blood through and they just get worse and worse from there, you can try to roll the veins back in so it wont keep the blood cut off and swell more. the longer the vein is crimped off and not letting blood through, the worse and bigger and more you get untill you have surgery, At least this has been my experience, the hemorrhoid creams help the itching and wincing little pains.
