If it were a man?
The answer would be hemorrhoids, have you ever seen a fella try and pass a kidney stone? It’s the equivalent of labor so I’m told.
For me it would be the old piles too. Bugger kidney stones that’s agony.

lol i’ve changed my answer, i would rather have hemorrhoids being a guy, kidney stones would be painful as **** (how would you even get them out, ugggghhh)
